Terms & Conditions
By enrolling in our dance class, you agree to the following terms and conditions. Participants must arrive on time and wear appropriate attire for each session. We reserve the right to cancel classes due to unforeseen circumstances and will notify participants in advance if the participant has reserved a spot. Refunds will not be provided for missed classes, but makeup sessions may be scheduled at the instructor's discretion. Thank you for your understanding and cooperation!
I, [CLIENT NAME] hereby agree to the following:
1. I am fully aware of the risks and hazards involved in participating in a Cardio Grooves dance class
2. I understand that if I have any medical conditions, it is my responsibility to consult with a physician prior to and regarding my participation in a Cardio Grooves dance class
3. I voluntarily and expressly waive any claim I may have against Cardio Grooves and Thien-Linh Truong, for any illnesses, injury or damages that I may sustain due to my participation. In consideration for being permitted to participate in group or private dance classes and workshops provided by Cardio Grooves and Thien-Linh Truong, I agree to assume full responsibility and, on behalf of myself and, to the extent permitted by law, my spouse, heirs, executors, administrators, assigns, and other persons or entities acting or purporting to act on my behalf, hereby generally and completely release, acquit, and forever discharge and release Cardio Grooves, its owners, officers, directors, employees, instructors and representatives from any and all liability for any risks, illnesses, injuries or damages, known or unknown, which I might incur as a result of my participation.
4. I will not engage in any inappropriate conduct or conduct which is in violation of this Agreement of Release and Waiver which could result in injury or illness to myself or others.
5. I release the Cardio Grooves parties from any and all liability related to any loss, theft or damage of any of my personal property from the premises.
6. I agree that this Agreement of Release and Waiver shall be governed by the laws of Quebec, Canada, and if any conflict arises between me and Cardio Grooves, I will attempt to resolve the conflict in good faith through mediation before submitting my dispute to binding arbitration.
7. I have read the above release and waiver of liability, fully understand its contents, and voluntarily agree to all of the terms and conditions.
